Accessibility request forms

Accessibility Services uses a platform called Accommodate to securely store documents and manage student requests.

Many of our previous PDFs and Word documents have been migrated into Accommodate to streamline requests and services. Please log in to Accommodate from your MyAU portal under Resources > Support Resources. You will need your AU email address to log in to Accommodate.

New to Accessibility Services?

• Register - Complete the 2 step process
  • Section 1 is a Verification of Accommodation (online form) outlining your self assessment
  • Section 2 requires upload of medical verification of the functional impact of your disability from a medical professional. Medical Verification
  • Consult our summary of disability documentation page to understand accepted medical verification.
  • Once Section 1 and 2 are ready, submit the entire VOA form at one time.

Request Additional Accommodations and Services after you receive a Registration Letter.

• Log in to AU Accommodate with your AU e-mail. AU Accommodate Student Reference Guide
• Use the “Additional Accommodations and Services” form under menu item Accommodation;
  • To request a modification to your accommodation plan,
  • TO request assistance with CSG Service and Equipment grant applications,
  • To request a waived fee course extension for non Student Aid funded course,
    • For use only if the accommodation is in your accommodation plan. Otherwise see instructions in your student calendar).
    • Not for SFA funded students contact your SFA Specialist sfa@athabascau.ca
  • Request assistance with assistive technology

Alternative Format Course Material

• Log in to AU Accommodate with your AU e-mail. AU Accommodate Student Reference Guide
• Under Accommodation, submit the Alternative Format Material form
  • Only for those with confirmed accommodation for an alternative textbook will see the form
